The MD Logic Practice Building feature gives healthcare practices a powerful way to identify, reach, and reconnect with specific patient populations—helping improve follow-up care, strengthen patient relationships, and support long-term practice growth. With this feature, practices can quickly search for targeted groups of patients based on a wide range of clinical conditions, including but not limited to diabetes, thyroid disorders, hyperlipidemia, COPD, asthma, arthritis, Alzheimer’s disease, Parkinson’s disease, bipolar disorder, kidney disease, chronic GU conditions, chronic pain, and much more. The possibilities are virtually endless, giving practices the flexibility to build meaningful outreach campaigns around their own patient population and clinical priorities.

Identify and Communicate with Patients

In addition to condition-based searches, the MD Logic Practice Building feature can identify patients who have not been seen within a defined timeframe, such as the last 6 months, 12 months, or longer. Once the search criteria are entered, the system displays a list of patients who meet those requirements, allowing the practice to quickly begin outreach. Staff can then compose a personalized message such as: “It has been over 12 months since your last visit, and it is recommended that you be seen in our office each year to properly manage your elevated cholesterol. Please contact our office at [practice phone number] to schedule an appointment at your earliest convenience.”

Proactive Communication

This type of proactive communication does more than simply reconnect the practice with patients—it helps elevate patient care by encouraging patients to return for appropriate follow-up, ongoing monitoring, and continued management of chronic or long-term medical conditions.
